The bank where the family Trust is held, Bank of America needs a document that is added and notorized to both E and C's General Power of Attorney (Section 1.02 Default of Designation) with R and A as successor agents.  The director of accounts at Bank of America, informed me that this document would enable Robert and A to assist their parents ( E and C) with accounts (credit cards) outside the Trust.

Here is the language from the DPOA:

If no one is serving as my Agent under this power of attorney, E may appoint a successor Agent by written instrument acknowledged before a notary public and attached to this power of attorney.

Do you know what this is referring to?  has anyone done something like this?  Can you please share the template? What is this document called?
Thanks
